Identify the equipment
Provide clear photographs of the nameplate and the driven machine. Record whether the motor is AC or DC, its rated voltage and power, and any available model or serial number.
Describe the failure
Explain when the problem began, whether it occurs at startup or while running, and whether there was water exposure, overheating or a previous repair. Report observations without opening energized equipment.
Agree the repair scope
Ask what inspection, repair and testing will be included. Rewinding is one possible intervention; the appropriate work depends on the condition of the motor and its application.
